Patients often use the terms interchangeably, but cataract surgery and YAG laser capsulotomy are two very different procedures — done at different times, for different problems. Here is a clear breakdown.

The short answer

Cataract surgery removes the cloudy natural lens from your eye and replaces it with a clear implant. YAG laser capsulotomy is a much smaller laser procedure done months or years later, to clear a thin membrane behind that implant if it becomes hazy.

What cataract surgery does

Cataract surgery is a keyhole operation, typically 15–20 minutes per eye, in which:

- The cloudy natural lens is broken up and removed

- A clear artificial intraocular lens (IOL) is placed inside the eye

- The thin capsule that held the natural lens is left in place to support the IOL

Cataract surgery is a one-off procedure for each eye — once the natural lens is gone, a cataract cannot come back.

What YAG laser capsulotomy does

After cataract surgery, some patients develop posterior capsule opacification (PCO) — a thickening of the capsule behind the IOL. YAG laser capsulotomy uses focused laser pulses to make a small clear opening in that capsule so light passes through cleanly again.

YAG laser is:

- A 5–10 minute clinic procedure per eye

- Painless and outpatient — no hospital admission

- Permanent (PCO cannot recur once the capsule is opened)

Do most people need both?

Not everyone who has cataract surgery goes on to need YAG laser. Around 1 in 5 patients develops PCO within five years of cataract surgery. Modern lens implants and refined surgical technique have reduced how often PCO happens.

How the recovery compares

- **Cataract surgery:** Blurred vision on day 1, clearer over 1–2 weeks, drops for 4 weeks, final refraction at 6 weeks.

- **YAG laser capsulotomy:** Blurred vision for a few hours, clearer within 24 hours, occasional short course of drops, review at 2–4 weeks.
